Youth Safety Incident Reporting Policy

ROC Youth Community Engagement (RYCE) is committed to maintaining a safe environment for youth, families, staff, volunteers, and community members. This policy establishes clear procedures for reporting, documenting, and responding to incidents that may affect the safety or well-being of youth involved in RYCE programs or services.

Purpose

The purpose of this policy is to ensure that:

  • incidents involving youth safety are identified promptly
     
  • concerns are documented accurately
     
  • appropriate steps are taken to protect youth
     
  • legal obligations and ethical responsibilities are met
     

Definition of a Youth Safety Incident

A youth safety incident may include, but is not limited to:

  • physical injury occurring during program activities
     
  • threats of violence or aggressive behavior
     
  • allegations or suspicions of abuse or neglect
     
  • harassment or bullying
     
  • medical emergencies
     
  • missing or unaccounted-for youth
     
  • mental health crises or suicidal ideation
     
  • exposure to unsafe environments
     
  • violations of program safety policies
     

Any situation that could reasonably place a youth at risk must be treated as a safety incident.

Immediate Response

When a safety incident occurs, staff must prioritize the immediate safety of all individuals involved.

Actions may include:

  • ensuring the youth is physically safe
     
  • contacting emergency services if necessary
     
  • providing first aid or basic support until help arrives
     
  • removing individuals from unsafe situations
     
  • notifying a supervisor or organizational leadership immediately
     

Emergency services should be contacted whenever there is an immediate threat to life or safety.

Documentation Requirements

All incidents must be documented as soon as possible after the situation is stabilized.

Incident reports should include:

  • date and time of the incident
     
  • location where the incident occurred
     
  • names or identifying information of individuals involved
     
  • description of the event
     
  • actions taken by staff
     
  • any witnesses present
     
  • follow-up actions or referrals made
     

Incident reports must be submitted to organizational leadership within the designated reporting timeframe.

Confidentiality

Information related to safety incidents must be handled with confidentiality and shared only with individuals who have a legitimate need to know.

Confidential information may only be disclosed:

  • to appropriate supervisory staff
     
  • to relevant authorities when required by law
     
  • to emergency responders or medical personnel when necessary
     

Follow-Up and Review

RYCE leadership will review incident reports to determine appropriate follow-up actions. These actions may include:

  • additional safety planning
     
  • referral to support services
     
  • communication with families or guardians
     
  • internal review of program procedures
     
  • policy improvements to prevent future incidents
     

Commitment to Continuous Safety

RYCE continually evaluates its safety procedures and training practices to ensure the highest possible standards for youth protection.

Staff and volunteers are expected to participate in training related to:

  • youth safety
     
  • crisis response
     
  • trauma-informed engagement
     
  • incident reporting procedures
